The location for the cabin was a ranch in California. Moreover, a lot of the interiors were filmed on sets built in a warehouse.
Special effects makeup was done by the newly formed KNB EFX, Nightwish was their second movie. KNB EFX are responsible for the special effects on The Walking Dead.

The ranch Nightwish was shot on was the Zorthian Ranch. The warehouse where where interiors were shot was next door to the warehouse where Chicago was practicing for their upcoming tour.
