The producers of this film asked the producers of Scary Movie 4 (2006) if they could use their bathroom set for this film as it was an exact replica of the sets used in Saw (2004) and Saw II (2005). They were given permission to use it.
As with the previous two films, only the actors who appeared in the final scene were given the complete script.
The film's most graphic scene - Jigsaw's brain surgery - remained completely uncut by the MPAA. The filmmakers argued that it was no different from what people would see in any medical documentary on TV.
The pig carcasses were made out of foam, rubber, and latex. However, the pig props were filled with real, live, disinfected maggots. Tobin Bell said that, out of all of the traps in the series, this one, known as the "Pig Vat", was his favorite.
During the brain surgery scene prop master James R. Murray had to hold Bahar Soomekh's arms still for the close-up shots because she was unable to steady herself and keep the tool under control.
"Dr. Lynn Denlon: [after drilling holes into his skull non-anesthetized] John, how're you doing? Jigsaw: Never better."
"Jigsaw: [after Amanda gets shot] Amanda, It's OK. This was your test. Your game. I was testing you. I took you in. I selected you for the honor of carrying on my life's work. But you didn't. You didn't test anyone's will to live. Instead you took away their only chance. Your games were unwinnable, your subjects merely victims. In my desperation I decided to give you one last chance. So I put everything in place. You didn't know that Lynn and Jeff were husband and wife. I had to keep that from you for the purposes of my game. I had to leave out the ruined marriage, the cheating wife, the vengeful husband, the neglected daughter, and I let you make your own choices. I wanted you to succeed... You couldn't.
