The film follows the life of Kenji Onishi, a boy who has dreamed of becoming an idol since childhood. However, he faces a deep identity crisis and the fear of judgment from others. Bullied at school and unable to confide in his family, Kenji finds a sense of belonging among the exuberant performers of a cabaret bar, where he begins to work without his parents' knowledge. On stage, he debuts with a new identity: Ai. His life takes a turn when he meets Dr. Koji Wada, a doctor tormented by guilt for not having been able to save his patients in the past. He empathizes with Ai's malaise and decides to challenge the taboos of the time by dedicating himself to research on gender reassignment surgery. Despite social prejudices and his own inner doubts, Dr. Wada chooses Ai as his first patient for the intervention.

The film is based on the autobiography of Ai Haruna, 'Subarashiki, Kono Jinsei', and Dr. Koji Wada's non-fiction book 'Penis Cutter: The Doctor Who Saved Those With Gender Identity Disorder'.
Haruki Mochizuki was selected for the lead role through an audition.
