Felicity Huffman had to stay in character all day while shooting. This included having to keep Bree's voice on so that she wouldn't lose it. If she turned it "off" she would lose the voice.
Not knowing the film would be such a success, most of the wardrobe and props were donated to charity and thrift stores after production wrapped.
Many scenes involve improvisation. The scene where the characters are riding in the car towards sunset is one of them: the director (Duncan Tucker) asked Felicity Huffman to start poking Kevin Zegers in the stomach and shoulder. Kevin Zegers stayed in character, and the director liked the result.
The initial idea for the screenplay occurred in a conversation between screenwriter/director Duncan Tucker and intersex actress Katherine Connella. While discussing male/female perceptions, Connella shocked Tucker (they had been sharing a house for about four months at the time) with the revelation that she had been raised as a male.
Felicity Huffman had to finish shooting her scenes earlier than scheduled when she secured a role on Desperate Housewives (2004).

"Dr. Spikowsky: How do you feel about your penis? Bree Osbourne: [giving up] It disgusts me. I don't even like looking at it. Dr. Spikowsky: What about friends? Bree Osbourne: They don't like it either. Dr. Spikowsky: No, I mean do you have the support of friends? Bree Osbourne: I'm very close to my therapist. Dr. Spikowsky: What about your family? Bree Osbourne: My family is dead."
