IMDbThe part of Nate Tanner was originally supposed to be James Dalton, Patrick Swayze's character from the first Road House (1989), but Swayze backed out due to creative differences, and the character was changed.
The ventilator was a real working ventilator. An artificial lung was placed off camera to make it work.

The twins girls who are seen with Shane, the main character, on the front of the "Road House 2" DVD cover are implied to be supporting characters in the film. These woman are only seen as extras leaving the Black Pelican bar in the film's opening scene, and neither of them have any dialogue.
Sherri(Crystal Mantecon), the lingerie-clad DEA agent who flirts with Shane at the beginning, never had her name mentioned in the film.
