In the original script by Dario Poloni, the second half of the film turned to be entirely supernatural: Langiva was revealed to be the reincarnation of the Devil, while Osmund "was actually in Hell and Hell being in the physical place."Christopher Smith rewrote the second half, changing these elements to represent the idea that to him "Hell is the Hell you are in within yourself."
Sean Bean was attached to the film from the start, which helped enormously in raising the budget.
The Black Death is also known as the Pestilence, the Great Plague, the Plague or the Black Plague and resulted in the death of around 75 to 200 million people in Eurasia (the whole land mass of Europe and Asia combined), peaking in Europe from 1347 to 1351 and was contracted from fleas carried by rodents.
This is the second time Christopher Smith has made a film in Germany; parts of Creep were filmed there too. Despite being in English with an English cast and director, Black Death (2010) is essentially a German film.
Lena Headey was going to play Langiva, but was replaced by fellow Game of Thrones (2011) actor Carice van Houten. Famke Janssen was also briefly attached to the role.
"Ulric: The woman was already dead, but I freed her. The mob would have held her again and burned her. I spared her suffering. Sometimes that is all you can do."
"Langiva: You're in mourning. Osmund: Is it so obvious? Langiva: Was it a woman? [Osmund is silent and looks away] Langiva: It's no sin to love a woman. Osmund: I am sworn to God. Langiva: A man needs more than divine love to sustain him. And he shouldn't be punished for wanting it."
