In order to prepare for their roles, several cast members watched a number of real autopsies.
Most of the organs seen in the movie came from pigs and cows.
The city was identified as Philadelphia in an earlier draft of the script.
(at around 38 mins) The poem Jake recites is "The Emperor of Ice-Cream" by Wallace Stevens.
Due to disturbing behavior throughout - including violence, gruesome images, strong sexual content - the first theatrical trailer of the movie was digitally altered. It showed operation scenes without human organs, also Milo Ventimiglia's hands digitally erased in a love scene. But the later Red Band version was released without any alterations. You mixed ethonol and nitrous oxide with the alcohol, probably a flavored alcohol to mask the bitter taste. Using a needle would've been too dangerous, because any first year med student would know to look for the mark. You cut him up while he was still alive, but unconscious. So there was no struggle. Then you put him in the back of your truck, drove him to 4th Avenue, and shot him in the back of the head. Jake Gallo: Motherfucker. Well, you are good. [Ted starts to walk away] Jake Gallo: Hey, you want to play the game, you gotta bring something to the table. Comprendo?"
