Director Harry Lighton spent a weekend with real members of the Gay Bikers Motorcycle Club, who later served as advisors for 'Pillion' and appeared in the film as members of the gay biker gang. The members of the GBMCC also attended the film's premiere at the 2025 Cannes Film Festival.

Harry Lighton said he cast Alexander Skarsgård to play Ray because he needed "a six-foot tall blonde guy".

Jake Shears's film debut. Casting director Kahleen Crawford had seen Shears in the musical Cabaret and they became friends. Some time later, Crawford called Shears inviting him for the film. Shears had been acquainted with Alexander Skarsgård through friends years before that and was excited to work with him in the film. Shears was not familiar with the gay biker and BDSM world, so he read the book The Leatherman's Handbook to prepare for his role. Shears said that Skarsgård and Harry Melling made him feel very comfortable in the orgy scene.
